This Privacy Policy explains how we collect, use, disclose, and safeguard information through our Lift Care App ("App"), which is used to manage non-medical home health care services for individuals with disabilities. The App collects and processes Health Data, as described in detail in this policy, to facilitate care coordination and service delivery.

2. Information We Collect

2.1 Account & Employment Information

  • Account information (name, email address, phone number, authentication credentials)
  • Employment information (role, certifications, work schedule, pay rates)
  • Communication records within the App (messages, incident reports)

2.2 Health and Care Data

Because our App is used to coordinate non-medical home health care services for individuals with disabilities, we collect and process the following health-related data:

  • Care recipient demographics: name, date of birth, gender, physical descriptors, emergency contacts, and legal guardian/authorized representative information
  • Care plans and goals: individualized care plans, therapeutic goals and objectives, protocols, and progress tracking
  • Medication support records: medication names, purposes, dosage information, administration routes, adverse effect indicators, choking/swallowing risk assessments, PRN (as-needed) medication details, and medication support event logs (reminders, observations, refusals, barriers encountered)
  • Shift and care documentation: caregiver shift notes describing care activities, goal/objective progress assessments with performance scores, support levels provided (independent, verbal prompts, physical assistance), and outcomes
  • Incident and safety reports: documentation of incidents including falls, injuries, behavioral events, near misses, and environmental hazards, with severity ratings and follow-up notes
  • Delegation of care tasks: records of clinical tasks delegated to caregivers, including delegating clinician identity and authorization periods
  • Health-related documents: uploaded care plans, health assessments, and other documents relevant to the individual's care

2.3 Location Data

  • GPS coordinates at shift clock-in and clock-out (to verify caregiver presence at approved service locations)
  • Service delivery location coordinates (for electronic visit verification compliance)
  • Mileage trip data including origin and destination addresses and coordinates (for transportation reimbursement)

2.4 Information Collected Automatically

  • Device information (device type, operating system, app version)
  • Usage data (features accessed, time spent in app)
  • Log and diagnostic data (errors, performance metrics, crash reports)

3. How We Use Your Information

We use the information we collect to:

  • Provide and maintain the App and our care management services
  • Coordinate care services for individuals with disabilities and their families
  • Track and verify caregiver shifts and services provided
  • Process payroll and manage employment records
  • Communicate with you about schedules, assignments, and important updates
  • Ensure compliance with applicable regulations and policies
  • Improve our services and App functionality

3.1 How We Use Health and Care Data

Health and care data described in Section 2.2 is used specifically to:

  • Coordinate and deliver individualized care services according to each person's care plan
  • Enable caregivers to document medication support, track care goals, and record shift activities
  • Facilitate communication among care teams, families, and guardians about a person's care
  • Document and respond to incidents to protect the safety and wellbeing of care recipients
  • Comply with state electronic visit verification (EVV) requirements and other regulatory obligations for home health care services
  • Generate claims and billing records for services rendered to applicable payers
  • Maintain audit trails of access to health data for compliance and security purposes

We do not use health and care data for advertising, marketing, or any purpose unrelated to the delivery and coordination of care services.

4. Information Sharing

We may share your information with:

  • Care Teams: Other authorized caregivers and coordinators involved in care delivery
  • Families and Guardians: Information relevant to the care of their family member, as appropriate
  • Service Providers: Third-party vendors who assist with App operations (hosting, analytics) under confidentiality agreements
  • Regulatory Bodies: Government agencies as required for program compliance and reporting

We do not sell your personal information to third parties.

Location data is collected only during active use of these features.

You can control location permissions through your device settings. Some features (such as shift clock-in) require location access to function properly and to meet state regulatory requirements.

9. Changes to This Policy

We may update this Privacy Policy from time to time. We will notify you of significant changes through the App or other appropriate means. Your continued use of the App after changes become effective constitutes acceptance of the updated policy.
