Lift Family Care exists to put the power and the pay back into the hands of families. By using technology, reducing overhead and digitizing the process, we pass more Medicaid dollars directly to the people doing the real work: Family Caregivers.

Are You Eligible?
We support participants on both the New Mexico Medicaid Developmental Disabilities Waiver (DD Waiver) and the self-directed Mi Via Waiver. You must have approval through the state for a Family Living Provider budget on the DD Waiver or In-Home Living Supports through Mi Via.
Are You Connected?
Care Teams need modern smartphones with a data plan and must be comfortable using basic apps. Teams must also be comfortable reviewing shift details digitally via smartphone or computer.
